 We are looking for the Patient Care Team!

-is responsible for managing the RPM (Remote patient Monitoring) & CCM (Chronic Care Management) and APCM (Advanced Primary Care Management) programs.

-Full Time:  9 am – 5 p.m. (Mon- Fri) at Our Clinic    

Job Duties: Enroll The program, Maintain Clinical Data, Communicate with patients

Preferred Bilingual: English & Korean
