The care coordinator is the backbone of Home Health Care. The Coordinator recruits and manages field employees who are direct care providers to patients. The Coordinator collaborates with RN and outreach team to arrange appropriate patient field employees. Care Coordinator embodies the mission and vision of this agency to field employees.

* Job Description and responsibilities
Ensure all patient cases are staffed and documented.
Handle and mediate conflict for clients and aides.
Support field employees to do their job at the highest level: As a team leader for employees, the coordinator listens, consults, and motivates employees
Prepare and maintain employee records to be compliant with Georgia Healthcare Facility Regulation.
Monitor and report all outstanding issues from employees
Ensure employee clock in/out
Maintains employee profile
Ensure employees’ benefits
Maintain patient confidentiality by adhering to State & Federal regulations.
Care Coordinator maintains on-call duty as needed.
Participate in the evaluation of employees
Participate in the orientation of field staff
Ensure employees perform their duties as instructed
Verify employee's work schedule and submit to payroll
Handle complaints /incident investigations and submits them to the Administrator
Perform other duties as assigned.

** Requirement
English and Korean
Reliable attendance and punctuality.
Excellent organizational skills with particular attention to detail
Excellent interpersonal skills with the ability to problem-solve/follow through.
Ability to handle multiple tasks efficiently and timely
Computer skills/knowledge
Bachelor’s degree
