Your claim for Pandemic Unemployment Assistance (PUA) has been processed by the Georgia Department of Labor (GDOL). GDOL is unable to verify your employment and wages with the Georgia Department of Revenue for calendar year 2019. Therefore, additional information is needed from you to determine your PUA eligibility. You must submit proof of your employment and wages for the tax year January 1, 2019 through December 31, 2019 within 21 days from the date of this email.

If you fail to respond timely and provide sufficient documentation, your eligibility will be reconsidered. You will be denied for future benefits and required to repay all PUA benefits received.

Documentation must be submitted to our secured email at PUA@gdol.ga.gov.

Acceptable proof of employment and wages includes, but is not limited to the following:

Proof of Earnings/Wages

 

• 2019 IRS Form 1040 and a copy of Schedule C, Schedule F or Schedule SE tax return
• 2019 State Income tax return
• 1099 documentation
• Pay stubs, earnings statements, or vouchers
• Bank records showing payroll direct deposits

Proof of Self-Employment

 

• Recent records connected to your business, such as bank records
• Phone, utility or insurance bills
• Sales tax return
• Current business license
• State or Federal Employer Identification Number
• Property titles, deeds, or rental agreement for your place of business
• Recent newspaper, phone book, or Internet advertisement for your business
• Statement(s) from recent customers
• Billing notices, invoices, or sales records

You will receive a PUA Benefit Determination, DOL-5159PUA, by mail. The Benefit Determination will show your PUA Weekly Benefit Amount and the number of weeks you are potentially eligible to receive PUA. If you disagree with your Benefit Determination, you have 15 days from the date of release to file an appeal.

Next Steps

PUA benefits are paid on a weekly basis. Start requesting your weekly benefits the first Sunday following the date of this email and every Sunday thereafter. You must request your PUA payments each week to receive payment by selecting dol.georgia.gov (https://dol.georgia.gov/gdol-covid-19-information).

Federal Pandemic Unemployment Compensation (FPUC) provides an additional $600 weekly payment to any individual eligible for PUA. FPUC is payable for week ending dates April 4, 2020 through July 25, 2020.
